Applicants, please note:
- Fill in every field of this application form and keep the information precise and short.

- The information in this form will first be assessed by all interested GDC experts working in programmes in partner countries on our internet platform. Next, it will be ranked (in relation to other proposals) by the BMZ Task Force Health. The proposals with the highest ranking will be documented in the GHPC.
- We will post the relevant sections (i.e. items 2, 4, 5 and 6 of the below form) of all proposals as soon as they have been submitted to us at http://german-practice-collection.org/en/new-proposals .
- We will contact you as soon as the BMZ Thematic Team has taken its decision. This is likely to be at the end of January/start of February 2013.

5. Why should this approach be documented in the German Health Practice Collection?
Describe in what ways this approach is a) innovative and conforms to the ‘state of the art’ in its field; b) contributes to raising the profile of German Development Cooperation in its field (refer to policy briefs, sector strategy papers and the GDC profile in health or social protection, respectively); c) meets the GHPC criteria of transferability to other contexts, participation and empowerment, gender-sensitivity, sustainibility
6. What evidence on outputs and outcomes is available to show that this approach is effective and that it has been scaled up to cover a significant proportion of the partner country’s population?
List all internal monitoring and evaluation reports, external evaluations and other studies that provide evidence for the effectiveness of this approach. Also, please explain how long this approach has been implemented and to what level/coverage it has been scaled up.
7. What kind of photo and film material is available and can be used for the documentation?
Please note that – should your programme be selected for documentation – you will be requested to provide 15 to 20 high resolution photographs of your programme context and your programme in action.
8. Which didactic or operational materials (manuals, guides, IEC materials, protocols) or similar can be made available for downloading from the online toolbox that will accompany the publication of this approach?
9. Which itinerary and schedule would you recommend for the writer who will visit your programme to do the necessary journalistic research for this GHPC report? Whom should he or she meet and interview? Which sites should he or she visit?
Give an indication of the places the writer charged with the documentation of this approach should visit, interviews to be done, the persons who should be interviewed and sites/events/activities that should be visited/witnessed and documented.
